Driving Change Where It Matters Most

Our work is grounded in four key priorities: improving access to care, advancing health equity, innovating and specializing services, and building a sustainable future. Together, these commitments reflect a bold vision for change, one that uplifts the dignity of every person and ensures no one is left behind when it matters most.

IMPROVING ACCESS TO CARE

Many structurally vulnerable individuals face significant barriers to receiving healthcare including inequitable access to public funding. We are committed to expanding access to sustainable care by sharing our expertise through consultation, collaborating to co-design and unify specialized programs and services, and empowering non-healthcare workers to advocate for and support individuals in all the places they call home.

ADVANCING HEALTH EQUITY

Structural inequities in healthcare disproportionately impact people experiencing homelessness, mental health challenges, substance use disorders, involvement with the justice system, disabilities, and those in Indigenous communities. These individuals often face stigma—even within healthcare settings. We are dedicated to changing the narrative about the dignity and worth of every person while working to eliminate systemic barriers to equitable end-of-life care.

INNOVATING AND SPECIALIZING CARE

End-of-life care must be tailored to the diverse, complex and holistic needs of structurally vulnerable populations. We are committed to co-designing, developing and implementing specialized, evidence-based models of care that address unique challenges and improve patient outcomes.
